To reach a conclusion is to arrive at a settled judgement after thinking your way there. The jury files back in; the report finally says what it thinks.
Reach is the verb of arrival, so the phrase carries the whole journey with it — you reach one at the end of a process, never in the first minute. It takes about or on: they reached a conclusion about the cause. Come to is the everyday swap, and draw is the near neighbour that means something else: you draw one from evidence, you reach one after weighing things up.
